Exceptional Family Member Program (EFMP)

What is EFMP?

The Exceptional Family Member Program (EFMP) is a mandatory enrollment program for military families with special needs, offering a range of support and services. It helps coordinate housing, education, and medical care, and ensures these considerations are integrated into military assignment decisions.
 
Family members must undergo screening and enrollment, if eligible, before a soldier's accompanied tour to an OCONUS location where family travel is authorized. This includes a medical records review for all family members and developmental screening for all children 5 years of age and younger.

There is no face-to-face or telephone inquiry service during this time, but customers with previously scheduled appointments will be seen. Please note that in order to provide the most accurate and individualized processing, all EFMP encounters (OCONUS screens, enrollments, disenrollments, etc.) are by appointment only.

What Are the Initial Enrollment Steps?


Please review the Initial/Update Medical Enrollment Form to view the necessary steps to take to enroll in EFMP.

Additionally, you will need to use DD Form 2792, Exceptional Family Member Medical and Educational Summary and DD Form 2792-1, Exceptional Family Member Special Education/Early Intervention Summary to complete enrollment submission. 

Note: If you're having trouble viewing the DD Form 2792 and/or DD Form 2792-1, follow these steps:
  1. Save the form to your computer by right clicking the form's link and selecting "Save Link As."
  2. Open the saved form with Acrobat Reader and click "Enable All Features"
  3. Click on "Enable All Features" in the yellow bar at the top to view the form

How to complete command sponsorship

All military dependents traveling outside the continental United States, including Alaska and Hawaii, must undergo a physical within 19 months before travel. Review E-EFMP Command Sponsorship Process for more details on the command sponsorship process.
